For example, a service manager may organise incident response while a business director is accountable for the service.
An interview answer to “who runs this?” can supply an operational owner. Accountability needs an explicit statement or supporting record. If no accountable person is recorded, the view shows a gap in the record, not proof that nobody is accountable.
What to do. Open Accountability, check the source beside each owner, and ask the responsible team to confirm who is answerable. Have an authorised writer record the clarification through the supported capture or import path.
